On checking Mr. Jones’ medication chart, you find a valid order for the following:
• Paracetamol tablets 500mg to 1000mg orally every 6 hours.
You decide to administer 1000 mgs of Paracetamol. In the drug cupboard is a stock of 500 mg tablets.
In your answer, provide the working out of the dose you would administer and discuss two (2) medication safety issues and two (2) legal nursing precautions you should consider prior to the administration of this drug.

3. Discharge Planning
Discharge planning often involves extensive patient education in relation to the post-operative recovery period. Complete the discharge template that has been provided and attach this to your submitted paper, as an appendix. You are then required to discuss the rationale for the instructions you have given Mr. Jones and his family which address his post-operative management goals. Aspects to consider could be, ambulation, return to work, pain relief, medical follow up as well as any other issues that maybe relevant to Mr. Jones, his needs and his family’s needs.

Post- operative pain assessment

Definition of operative pain

An effective management system in postoperative care is the basic need of any individual patient. The medications and other prescriptions can be managed in postoperative pain. According to the methods in humans, there is no gold standard path that exists to assess this topic. The acceptance of this concept’s widespread improvement shows that the last decade of pain was evaluated by the surgical wards. The nurses and the doctors systematically handle or lead this evaluation which occurs only with the observational scales that a patient himself noticed. However, while describing the pain definition one should know about all types of pain, such anticipative pain, psychogenic pain, mixed category pain and many more.

Strategies for change and improvement

Postoperative care nowadays is upgraded and it is very much accessible too. Pain can be signified as subjective feelings. The 1999 audit decided to explain a mandatory training or a program that can implement postoperative pain, and all the ward nurses, staff, surgeons were there included (Li, Ma & Xiao, 2019). Acute pain, chronic pain, pain, neuropathic pain, functional pain and many more are there. It has been suggested that for any type of pain first one needs to seek medical attention. The pain can be described in so many dimensions like an ultimate intensity, location, consequences that stands for the emotion and correlates

Pain management strategy

The strategy of medicines, physical massage or any therapies can manage the pain. The medicines and complementary therapies are also included in this strategy. According to the studies, a psychologist can help a person to destroy the pain by discussing the outlook and the emotional barriers of his life (Scarborough & Smith 2018). This also can work as a strategic skill. Some studies suggest that a bad experience can be the cause of pain and for that first need to understand the cause and make effective ways to reduce the bad experience and surprisingly pain can be improved from the person’s life. Here are some keys or strategies that can relate to overcome the pain from someone’s life- need to know the exact medicine for the pain, techniques to relax the body and the mind too, a group of community that can support the patient to overcome with communication skill.

The process of discharge planning is the combination of many tasks as it helps in quick identification and early assessment of patients and describes the procedure of discharge of patients. The process of discharge planning contains much valuable information about the type of care that has been taken by the nursing authorities (Satiawati, 2020). This also includes not only the nursing authorities but also the hospital authority is responsible to take care of the patients and making proper Discharge planning. It is necessary for the nursing department they should take proper care and follow some techniques before discharging Mr. Jones

Develop a clinical plan at the time of admission: At the time of taking admission of Mr Jones, the admission may be taken by the senior and experienced management team. So the management may implement and develop an appropriate plan within 1 day to focus on the whole health condition of the patient. A critical plan helps the patient in planning various aspects of care required to discharge. 

Evaluation and assessment: Mr. Jones if in a stable condition he must be discharged after proper evaluation assessment by the nursing departments. Difference checkups are also needed to be done before discharging the patients and after proper evaluation and assessment of all medical checkups if authorities are of the mind that the patient’s condition is stable then they may discharge the patient (Lou et al., 2020). The guidance to the family of Mr. Jones is given according to the current conditions of his health and future care and medicine that is also needed to give to the patient and if possible hire a nurse to take care of the patient.

The main objective of nursing documentation is to maintain a database in which all the details required data regarding files is included and for this useful plan is maintained for the care of patients. In the report of Mr. Jones, a suitable technique of nursing documentation may be chosen as there are various types of nursing documentation such as Nursing program notes, Graphic Sheets, Narrative nursing notes and many more. It is necessary that in the process of documentation of Mr. Jones all the necessary disclosures have been provided in the computerised documentation process as it manages a huge volume of important information (Akhu‐Zaheya & Bany Hani, 2018). In the process of nursing documentation, different kinds of databases and plans of care were taken by the nursing authorities of the patient Mr. Jones. The database maintained in the nursing documentation should also contain various pieces of information regarding the discharge of the patients. Like the number of cares taken by the nursing department and physician’s role in taking care of patients. It is also disclosed in the documentation that the weekly progress report is also has been done by the nursing department and that assures appropriate care has been taken by the authority before making discharge orders of patients. Some of the relevant disclosures that have been provided in the nursing documentation include the resolved health issues Self-care activities discharge destinations and the comfort level of the patient Mr. Jones.
