You are employed as a massage therapist at Treatment Plus. Today is a busy day as you have 3 new clients booked in for treatments for the morning. Before each treatment you will need to sit down and have an initial discussion on their individual health and needs for treatment. Each individual has completed a health assessment.

You have been working in a 9-5 office job for the last 20 years of your life. Your job involves you sitting at the desk all day without much time to get up and walk around or take a break. The nature of your job is quite stressful so you often work late hours. You don’t have much time to exercise or keep fit and have a generally poor diet. You tend to slouch over while working at your desk and have always worked like that since early adulthood. The pain in your upper back, shoulders and neck is becoming worse, so much that you are even developing headaches which you assume are related. You are a side sleeper so wonder if this is also a cause for the shoulder pain. You have seen a massage therapist a long time ago for similar issues which was beneficial, and stopped going when the pain subsided. You are seeing an osteopath for treatment once a week which is not relieving much of the tension. If the massage treatment works you will consider ceasing osteopathy sessions. You do not take any prescribed medication. |
New Client 2 – the training athlete profile
You train almost 5 days a week and are quite fit. Your body is fairly used to minor aches and pains however a month back at training you think you pulled a muscle or ligament in your right leg. You tried to rest it immediately and it felt okay at the time, however because you did not want to stop training you recently aggravated the pain again. You tried to stretch it out and take standard pain killers to ease the pain which works most of the time, although you want a long term solution to the problem. It hurts most when you try to bend or stretch it and seems to be really painful first thing in the morning. The cold air makes it feel worse as well so you try to keep covered up and are considering taking a break from training until it subsides. If massage doesn’t help, your next option is to try a physiotherapist. |

It dates back to 3000 BCE in India. Massage therapy was considered as a sacred system of natural healing. Mainly used by Hindus in Ayurveda. Massage therapy is an activity that was often passed down through generations for the purposes of relieving pain, heal injuries, and to cure certain types of illness. Massage was believed to restore the natural and physical balance of the body. In 2700 BCE , the healing methods of massage reached China. In China the methods grew as a skill and a practice of traditional medicine. The Chinese method was similar to the one practiced in India, since all of them believed that diseases were caused by body imbalances. Through massage and other body works methods, energy will flow through the pathways, enabling the body to heal naturally. Massage therapy made its way to Egypt By 2500 BCE, where it was captured in tomb paintings. The Egyptians introduced their own body techniques and developed reflexology. Reflexology entails application of pressure in specific joints. The Monks studying Buddhism in China brought the idea of massage therapy to Japan in 1000 BCE. Japanese put a twist on massage therapy and later called it Shiatsu. This method is designed to strengthen organs through stimulation of pressure points to rebalance energy levels. |

Massage therapy is a scientific manipulation of the soft tissues of the body by use of the surface of the hands to put pressure on a certain part of the body to produce effects in the body systems like the nervous system, muscular system and the general circulation of blood. |

Explain what is meant by ‘Swedish massage’.
This is a massage technique that is a gentle type of body massage, aimed at relaxing the entire body into a state of bliss. A massage therapist uses a long smooth strokes, often done in the direction of blood flow towards the heart. Also the therapist uses kneading and perform circular movements with the help of a lubricant, either a lotion or oil. This type of massage is highly recommended to those who are new to massage, experience a lot of tension and are sensitive to touch. This massage helps and individual relax their muscles and remove tension. It also helps in boosting blood circulation throughout the body. |

Explain the following stroke techniques used in Swedish massage.
Effleurage | It is the most superficial stroke. It is a long gliding stroke often used as an opening stroke in in massage mainly to apply the lubricant to assess the tissues of the body. It is applied to all areas of the body with an open palm or even a soft fist. It soothes and relaxes the mind and the body by triggering the nervous system. |
Petrissage | The act of kneading the body muscles in a rhythmic manner of lifting and squeezing fleshy areas of the body. The movement is slow but continuous in order to loosen muscles and boost blood circulation. |
Friction | This involves deep movements that often cause layers of the tissues to rub each other. Superficial; friction is applied close to the joints, spine and any other bony parts. It is done by use of finger tips to create a deep circular movement backwards and forwards. This type of stroke helps in breaking adhesions that are formed when muscles are bundles together. |
Vibration | This is where the therapist gently shakes or trembles the flesh with the help of hands or fingertips. It helps in improving blood circulation in the muscles. It is very helpful to individuals with back pains. |
Percussion | The therapist uses rapid and repeated tapping strikes often with loose fist or cupped hand, mainly done on fleshy areas. This stroke is important in reviving overworked muscles. |
Passive and Active Movements | Involves stretching the muscles and connective tissues often done by pulling the head, leg and arms. It helps in improving joint’s flexibility by stretching and lengthening the tissues and muscles. |

It is characterized by the use of assessments and tests to identify structures causing the pain or dysfunction. Once the cause of the pain has been established, the massage will be tailored to focus strictly around treating the muscles causing the pain in the body. It can be said, remedial massage is tailored to each individual unlike relaxation massages. Remedial massages targets specific muscles, using techniques like trigger point therapy and muscle stripping. The pressure in remedial massage is often deeper than in relaxation massage. This massage is tailored to individuals have chronic pains or experience muscle tension. Relaxation massage The intention is to de-stress the body, calm and relax the mind and the entire body as well. It does not focus in a specific part in the body like in remedial massage. This massage focuses on the mental and emotional health, because it calms the mind and the nervous system. Relaxation massage loosens up muscles to encourage blood flow in the body and boosting immune system through stimulation of lymphatics. This massage is designed for individuals having stressful weeks or days. It involves smooth strokes and kneading at a chosen pressure. |

You are employed as a massage therapist at Treatment Plus. Today is a busy day as you have 3 new clients booked in for treatments for the morning. Before each treatment you will need to sit down and have an initial discussion on their individual health and needs for treatment. Each individual has completed a health assessment.

The nature of your job is quite stressful so you often work late hours. You don’t have much time to exercise or keep fit and have a generally poor diet. You tend to slouch over while working at your desk and have always worked like that since early adulthood. The pain in your upper back, shoulders and neck is becoming worse, so much that you are even developing headaches which you assume are related. You are a side sleeper so wonder if this is also a cause for the shoulder pain. You have seen a massage therapist a long time ago for similar issues which was beneficial, and stopped going when the pain subsided. You are seeing an osteopath for treatment once a week which is not relieving much of the tension. If the massage treatment works you will consider ceasing osteopathy sessions. You do not take any prescribed medication. |

You train almost 5 days a week and are quite fit. Your body is fairly used to minor aches and pains however a month back at training you think you pulled a muscle or ligament in your right leg. You tried to rest it immediately and it felt okay at the time, however because you did not want to stop training you recently aggravated the pain again. You tried to stretch it out and take standard pain killers to ease the pain which works most of the time, although you want a long term solution to the problem. It hurts most when you try to bend or stretch it and seems to be really painful first thing in the morning. If massage doesn’t help, your next option is to try a physiotherapist. |

I have been feeling pain in my upper back, neck, and shoulders (demonstrating by hands) for some years now. I sought treatment from the osteopathist, but it is not helping much. It is even affecting my work.

Okay, I understand where the problem started. Back pains often arise due to several reasons; lousy sitting posture, heavy lifting, or even sitting down too much. How is your sitting posture in the office?

I often sit while slouching over, and that is how I have been sitting ever since I started working.

Definitely Kennedy. Most upper back pain is always triggered by slumping over a desk or a computer, and it is linked to the shoulder and neck. That is why you are feeling the same pain in those areas. I recommend trigger point therapy as a treatment for this kind of problem. Because in most cases, upper back pain can be a result of knots growing in those areas between the shoulder blades. This type of massage often targets the knots, where the pain is coming from. Applying pressure on those specific areas, we will be treating the real problem and the cause of the problem.

I prefer Shiatsu
