Role play: New client assessments
You are employed as a massage therapist at Treatment Plus. Today is a busy day as you have 3 new clients booked in for treatments for the morning. Before each treatment you will need to sit down and have an initial discussion on their individual health and needs for treatment. Each individual has completed a health assessment.

During your discussion, you will need to:

§ Assess their physical state identifying areas for treatment

§ Identify individual needs for treatment

§ Determine a massage style and recommend a treatment regime suitable to their needs.

Refer to the sample health assessments for each individual that are provided below.

You must cover the following discussion points during your initial health discussion with each client:

  • Explain the benefits of the treatment type.
  • Adjust communication to suit individual needs.

  • Deliver key messages at appropriate pace.
  • Gain the support and approval of recommended treatment throughout the discussion.
New Client 1 – the corporate worker profile

You have been working in a 9-5 office job for the last 20 years of your life. Your job involves you sitting at the desk all day without much time to get up and walk around or take a break. The nature of your job is quite stressful so you often work late hours. You don’t have much time to exercise or keep fit and have a generally poor diet. You tend to slouch over while working at your desk and have always worked like that since early adulthood. The pain in your upper back, shoulders and neck is becoming worse, so much that you are even developing headaches which you assume are related. You are a side sleeper so wonder if this is also a cause for the shoulder pain. You have seen a massage therapist a long time ago for similar issues which was beneficial, and stopped going when the pain subsided. You are seeing an osteopath for treatment once a week which is not relieving much of the tension. If the massage treatment works you will consider ceasing osteopathy sessions. You do not take any prescribed medication.

New Client 2 – the training athlete profile

You train almost 5 days a week and are quite fit. Your body is fairly used to minor aches and pains however a month back at training you think you pulled a muscle or ligament in your right leg. You tried to rest it immediately and it felt okay at the time, however because you did not want to stop training you recently aggravated the pain again. You tried to stretch it out and take standard pain killers to ease the pain which works most of the time, although you want a long term solution to the problem. It hurts most when you try to bend or stretch it and seems to be really painful first thing in the morning. The cold air makes it feel worse as well so you try to keep covered up and are considering taking a break from training until it subsides. If massage doesn’t help, your next option is to try a physiotherapist.

New Client 3 – the senior profile

When you were younger you were involved in an unfortunate car accident, which left you with an aching back and sore arm. You underwent rehabilitation to strengthen the muscles and movement for a period afterwards which helped relieve the extent of the pain. Most of the years after the accident you have learned to cope with flare ups from time to time however you have noticed the pain in both the lower back and left arm is getting worse. You have been retired for some time and are not overly active physically, though you do attempt to go for strolls when the pain doesn’t seem so bad. But lately, it is hard for you to take even just a few steps. You are seeing other health therapists, as you feel the relaxation of aromatherapy to calm your high blood pressure and see a reflexologist for the arthritis in your hands. You prefer a softer pressure applied to the areas of pain on your body.

Question 1

In approximately 250 words, provide an overview of the evolution of massage therapy.

It dates back to 3000 BCE in India. Massage therapy was considered as a sacred system of natural healing. Mainly used by Hindus in Ayurveda. Massage therapy is an activity that was often passed down through generations for the purposes of relieving pain, heal injuries, and to cure certain types of illness. Massage was believed to restore the natural and physical balance of the body. In 2700 BCE , the healing methods of massage reached China. In China the methods grew as a skill and a practice of traditional medicine. The Chinese method was similar to the one practiced in India, since all of them believed that diseases were caused by body imbalances. Through massage and other body works methods, energy will flow through the pathways, enabling the body to heal naturally. Massage therapy made its way to Egypt By 2500 BCE, where it was captured in tomb paintings. The Egyptians introduced their own body techniques and developed reflexology. Reflexology entails application of pressure in specific joints. The Monks studying Buddhism in China brought the idea of massage therapy to Japan in 1000 BCE. Japanese put a twist on massage therapy and later called it Shiatsu. This method is designed to strengthen organs through stimulation of pressure points to rebalance energy levels.

Question 2

In your own words, provide a definition of ‘massage therapy’.

Massage therapy is a scientific manipulation of the soft tissues of the body by use of the surface of the hands to put pressure on a certain part of the body to produce effects in the body systems like the nervous system, muscular system and the general circulation of blood.

Question 3

Explain what is meant by ‘Swedish massage’.

This is a massage technique that is a gentle type of body massage, aimed at relaxing the entire body into a state of bliss. A massage therapist uses a long smooth strokes, often done in the direction of blood flow towards the heart. Also the therapist uses kneading and perform circular movements with the help of a lubricant, either a lotion or oil. This type of massage is highly recommended to those who are new to massage, experience a lot of tension and are sensitive to touch. This massage helps and individual relax their muscles and remove tension. It also helps in boosting blood circulation throughout the body.

Question 4

Explain the following stroke techniques used in Swedish massage.

Effleurage It is the most superficial stroke. It is a long gliding stroke often used as an opening stroke in in massage mainly to apply the lubricant to assess the tissues of the body. It is applied to all areas of the body with an open palm or even a soft fist. It soothes and relaxes the mind and the body by triggering the nervous system.
Petrissage The act of kneading the body muscles in a rhythmic manner of lifting and squeezing fleshy areas of the body. The movement is slow but continuous in order to loosen muscles and boost blood circulation.
Friction This involves deep movements that often cause layers of the tissues to rub each other. Superficial; friction is applied close to the joints, spine and any other bony parts. It is done by use of finger tips to create a deep circular movement backwards and forwards. This type of stroke helps in breaking adhesions that are formed when muscles are bundles together.
Vibration This is where the therapist gently shakes or trembles the flesh with the help of hands or fingertips. It helps in improving blood circulation in the muscles. It is very helpful to individuals with back pains.
Percussion The therapist uses rapid and repeated tapping strikes often with loose fist or cupped hand, mainly done on fleshy areas. This stroke is important in reviving overworked muscles.
Passive and Active Movements Involves stretching the muscles and connective tissues often done by pulling the head, leg and arms. It helps in improving joint’s flexibility by stretching and lengthening the tissues and muscles.

Question 5

Explain the key features of the following treatment and how they are used in conjunction with massage therapy.

Treatment Key features How is this used with massage therapy?
Aromatherapy Aromatherapy is characterized by use of scented essential oils or massage oils, and plant extracts to promote health and well-being. It works through sense of smell or skin absorption. Aromatherapy is a type of therapy that incorporates the application of scented oils into a massage. The massage entails the application of gentle and harder pressure while using specific type of oil essentials. The essential oil is often diluted with a massage lotion and applied during massage. The client benefits from direct contact with the essential oil believed to have specific healing power when applied or inhaled. Aromatherapy is often added to traditional massage session as an extra service.
Reflexology Body is divided into longitudinal zones from head to toe.

Mainly focuses on the hands and feet by applying pressure to these areas that are connected to different parts of the body.

It is assumed that certain areas in the hands or feet act as sensors for other parts of the body

Reflexology entails applying some pressure to certain parts of the feet and hands to provide impacts on the health of related parts of the body. Each part of the pressure acts like sensors on the feet and hands which are linked to different parts of the body. This method is used as a complementary therapy, involves a mixture of massaging, squeezing, pushing and pulling.

Question 6

Provide 3 benefits of massage on pain and circulation.

Pain 1. Massage therapy relaxes painful muscles, tendons and body joints
2. Massage therapy helps in closing the pain gate through stimulation of competing nerve fibers. Thereby impeding pain messages to and from the brain.
3. Relieves stress and anxiety
Circulation 1. Massage relaxes the tensed muscles in the body, opening up blood vessels
2. Massage stimulates the nerve receptors causing blood vessels to dilate, thus facilitating blood flow
3. Through the squeezing effect of muscle contractions, massage helps in the movement of lymph

Question 7

Explain the key differences between ‘remedial massage’ and ‘relaxation massage’.

Remedial massage

The intention is to remedy the body. This massage is based for the entire body, with a focus of treating pain, dysfunctions and rehabilitation of injuries. It is characterized by the use of assessments and tests to identify structures causing the pain or dysfunction. Once the cause of the pain has been established, the massage will be tailored to focus strictly around treating the muscles causing the pain in the body. It can be said, remedial massage is tailored to each individual unlike relaxation massages. Remedial massages targets specific muscles, using techniques like trigger point therapy and muscle stripping. The pressure in remedial massage is often deeper than in relaxation massage. This massage is tailored to individuals have chronic pains or experience muscle tension.

Relaxation massage

The intention is to de-stress the body, calm and relax the mind and the entire body as well. It does not focus in a specific part in the body like in remedial massage. This massage focuses on the mental and emotional health, because it calms the mind and the nervous system. Relaxation massage loosens up muscles to encourage blood flow in the body and boosting immune system through stimulation of lymphatics. This massage is designed for individuals having stressful weeks or days. It involves smooth strokes and kneading at a chosen pressure.

  • Complete a health assessment check using the health assessment form.
  • Ask questions to assess client needs.
  • Identify and confirm areas of physical pain.
  • Provide a treatment option according to needs.

CLIENT ONE

Massage Therapist: Good afternoon Sir, Welcome to Treatment Plus

New Client: Good afternoon, how are you doing?

Massage Therapist: I am doing well; thank you. What about you?

New Client: Not so well.

Massage Therapist: Kindly have a seat. How can I assist you?

New Client: I had called earlier to book an appointment. My name is John Kennedy.

Massage Therapist: Welcome, John Kennedy.

New Client: Thank you so much.

Massage Therapist:

Allow me to check you in (Takes a check-in book and a pen). Your record indicates that you have been having persistent pain in the upper back, shoulders, and neck. Also, you have been visiting an osteopathist.

New Client:

Yes, you are right. I have been having persistent pain in those areas, and they became worse over the last months. I have been seeing an osteopathist once in a week.

Massage Therapist:

Now, please explain to me the problems you have been feeling in your neck, shoulder, and upper back.

New Client:

I have been feeling pain in my upper back, neck, and shoulders (demonstrating by hands) for some years now. I sought treatment from the osteopathist, but it is not helping much. It is even affecting my work.

Massage Therapist:

Okay, I understand where the problem started. Back pains often arise due to several reasons; lousy sitting posture, heavy lifting, or even sitting down too much. How is your sitting posture in the office?

New Client:

I often sit while slouching over, and that is how I have been sitting ever since I started working.

Massage Therapist:

Well, I see the cause of the problem. You must be feeling so much pain. Don’t worry; we will find the right solution for that problem so that you go back to feeling normal again.

New Client:

I believe you guys have the right solution for this issue (laughing).

Massage Therapist:

Definitely Kennedy. Most upper back pain is always triggered by slumping over a desk or a computer, and it is linked to the shoulder and neck. That is why you are feeling the same pain in those areas. I recommend trigger point therapy as a treatment for this kind of problem. Because in most cases, upper back pain can be a result of knots growing in those areas between the shoulder blades. This type of massage often targets the knots, where the pain is coming from. Applying pressure on those specific areas, we will be treating the real problem and the cause of the problem.

New Client:

I prefer Shiatsu

Massage Therapist:

Based on the description of the pain you are experiencing, I recommend trigger point therapy as a solution to this problem. I believe you applied Shiatsu in your previous massages, but it never worked. Be assured this type of treatment will work for you this time around.

New Client:

You are right, and I used Shiatsu some years back; it never worked. I go with your suggestion (Laughing).

Massage Therapist:

Well, that agreed. Now I am going to take you through safety measures that have been implemented in this company. First, in case of fire, there are stairs in your upper right, next to the door. We use personal protection and safety equipment approved for such functions. We also bear full responsibility in case of an injury in this residence. We also maintain boundaries so that we are not drawn into the client’s issues, and we maintain professional rapport. In case of any suspicious activity by our staff, please feel free to report immediately. Finally, we have strict confidentiality disclosure, which you have already signed a copy. Client’s information is confidential; anything done here remains here.

New Client:

Thank you so much for taking me through your policies. Listening to you has already relieved some of the pain (laughing loudly)

Massage Therapist:

You are most welcome Mr Kennedy. Lucinda will be your therapist today. Here she is to let you inside the massage room. Enjoy and get well soon.

New Client:

Thank you.

Kindly describe the pain, where it occurs and how often.

New Client 2:

I have been having chronic right knee and leg pain, and it hurts more when bending, squatting or stretch. It gets worse early in the morning. I member I pulled a muscle when I was training. I immediately withdrew from training and the pain subsided. I later went back to training only to make things worse. Since then I have been operating with painkillers, but I want a permanent solution to this issue.

Massage Therapist:

How often do you train? And have you ever attended a massage therapy or any other therapy?

New Client 2:

I have been training five times a week. I have never visited a massage therapist. I have been surviving on painkillers all along.

Massage Therapist:

I feel like you should have visited a massage therapist immediately it happened. I see like it’s an injury that is reoccurring whenever you commence your trainings.

New Client 2:

That is what I realized last week and I decided to contact Treatment Plus, I hope it is not too late to salvage the solution.

Massage Therapist:

You are the right place; nothing is too late for us here at Treatment Plus. I can see the knee has not swollen, indicating it can be a muscle or ligament issue. It’s an injury that is reoccurring whenever you commence your pieces of training. I recommend you start sports massage therapy. Sport massage is a good option for you since this seems to be a repetitive injury to a muscle. This option increases the flexibility and performance to athletes. It is a good choice since you are likely to injuries, and this massage also will help avoid sports injuries. Sports reflexology will be incorporated in this treatment. Associated benefits for this type of massage are increased joint range of motion, reduce muscle tension, increased flexibility and decreased muscle spasms.

New Client 2:

The type of massage seems to be very painful from your description (laughing).

Massage Therapist:

No (laughing), this method incorporates the most relaxing soft tissue massage that you have never felt before.

New Client 2:

That is okay then.

Massage Therapist:

Well, that agreed. Now I am going to take you through safety measures that have been implemented in this company. First, walking surfaces are safe, so no slipping. In case of fire, there are staircases in your upper right, next to the door. All the products used are medically tested and proven. New Client 2:

Massage Therapist:

I will get a copy for you. Once again you are most welcome to Treatment Plus. Where the experience is healing.

Let me take you to your massage room, we start of the process. I will be your therapist.

Massage Therapist:

Thank you very much for you kindness.

Massage Therapist: Good afternoon Sir, Welcome to Treatment Plus

New Client 3: Good afternoon, how are you doing?

Massage Therapist: I am doing well; thank you. What about you?

New Client 3: Not so well.

Massage Therapist: Kindly have a seat. How can I assist you?

New Client 3: I had called earlier to book an appointment. My name is Peter.

Massage Therapist: Welcome, Peter.

New Client 3: Thank you so much.

Massage Therapist:

Allow me to check you in (Takes a patient’s book and a pen). Your file indicates that you have been having chronic lower back pains and also a problem in one of your arms. Just describe to me how you often feel when in pain, or rather explain to me how you are feeling now

New Client 3:

Thank you for your time, my daughter (coughing). I have suffered from this pain for so many years. I have been attending massage therapies, which have made significant improvements in my health. There are days I could not even move. This has been my life. I suspect this pain comes from the injury I had from the accident when I was still young.

Massage Therapist:

How was the accident? Was it fatal? Tell me what transpired.

New Client 3:

The accident was fatal, and I lost my only Uncle during that accident. I survived the accident by the grace of God, though I stayed in the hospital for three months. That is where my problems originated from.

Massage Therapist:

I am so sorry about that. It must have been hard for you in your youthful life. But look at you now, you are going to do better after this massage, that I am very sure.

New Client 3:

I have been surviving on massages, and they are of help to me since I commenced.

Massage Therapist:

Which therapies have you been attending to? How has the reception been?

Massage Therapist:

I have tried a number of massage therapies, from reflexology to aromatherapy. Right now, I need a more gentle massage that calms me down while feeling relaxed at the same time.

Massage Therapist:

Assessing your situation right now, considering the previous experience of massage therapy that you have already attended to. I recommend Swedish massage therapy for you. This a more gentle massage that focuses more on soft tissues. It promotes relaxation by releasing muscle tension. It loosens up tight muscles as a result of daily activities. Gentle strokes will be applied on your back, shoulders, and neck to ensure there is smooth flow of blood to the heart and brain. We will also focus on areas of particular concern, depending on your preference.

New Client 3:

Is it a gentle massage?

Massage Therapist:

Yes, it is a gentle massage. Here are the expectations of this massage. The massage will be of the entire body. You will be required to undress as I wait outside the room, and while massaging, you will be covered by a clean towel. The essential oils used are all approved, unless you are allergic to some oils and lotions.

New Client 3:

That is fine then, Swedish massage is okay

Massage Therapist:

I will take you through some of the health and safety issues implemented here. In case of fire, we have one exit on my right side, leading to staircases; we have also ensured that the surfaces are not slippery and very comfortable to walk over. Our therapists here will make use of protective gear / gloves not to be in contact with the client. We also do not allow a patients with wounds or rashes in their bodies to be given massages, as this may escalate the spread of the infection to other areas of the body. We maintain client boundaries at all times to safeguard the interests of the clients and that of the therapist. We also guarantee the confidentiality of the client’s information. Therefore privacy is critical.

New Client 3:

Thank you for the information, I feel at peace now. You cleared some of my worries that had already started piling up.

Massage Therapist:

You are most welcome once again to experience is healing. Allow me to walk you to your massage room where everything has been made ready for you. I will be your therapist, and expect the best from me.

New Client 3:

Thank you so much, let us go
